📝 Step 2: Have an Assessment
An assessor will visit (or sometimes call) to chat about what’s going on day-to-day and what kind of help would make life easier.
They’ll look at things like:
What you can do on your own
What’s getting harder
The supports that might keep you living safely at home
💡 Tip: It’s helpful to have a family member or friend with you — they can help you remember details and share how things are going.

After the assessment, you’ll get an Individual Support Plan — think of it as your personal roadmap to staying independent!
It lists:
The supports you’ll receive
How often you’ll receive them
Your goals for living well at home
This plan makes sure everything is tailored to you.
📬 Step 4: Receive Your “Notice of Decision”
Once your plan is reviewed, you’ll get a Notice of Decision from the government.
This letter confirms:
Whether you’re eligible for Support at Home
Your funding level (from Level 1 to Level 8)
